Thank you for becoming a One Health Family Member and seeking veterinary care for your dog(s) or cat(s).

As a One Health Family Member, you gain better access to veterinary care and other resources to keep your dog or cat living a healthy and happy life. We’re pleased to help you afford the necessary veterinary care for your beloved pet(s).

How do I use my Voucher Card?

Your Voucher Card looks like a debit or gift card. These are the steps to take.

  1. Make an appointment with a Veterinary Partner.
  2. Bring your Veterinary Care Voucher Card with you or the email with your membership information. 
  3. Always bring extra cash, check, credit card, and/or debit card with you to cover any extra costs on your bill.

Where may I use my Voucher Card?

Veterinary Partners are the only veterinary locations known to accept Voucher Cards as payment.

How much money is on my Voucher Card?

The card is loaded and activated with $250 when you first receive it. 

How often can I use the Voucher Card?

You may use it as many times as you want until March 31st or when you spend $250, whichever comes sooner.

Can I obtain a Voucher Card for each pet living in my home?

No. An eligible household may only receive one Voucher Card each year they are eligible.

How much money do I have left on my Voucher Card?

You can check your Card balance through SwipeIt.com's self-service page. Enter your card number, check the "I'm not a robot" box, and click on "GET CARD STATEMENT."

What happens if my pet dies before I can use my Voucher Card?

Contact the Voucher Program Assistant at (216) 920-3051 Monday through Friday from 9 am to 5 pm to let us know. Leave a message if prompted.

Can I have more money put on my Voucher Card?

No.

What do I do with my Voucher Card if it has no more money on it?

Throw it out. If you qualify next year we can send you a new Voucher Card.

 

If you lost or found a lost card, contact the Voucher Program Assistant at (216) 920-3051 Monday to Friday from 9 am to 5 pm to let us know. After hours, leave one short message with your name and phone number where you can be reached.

What if I don’t have my Voucher Card at my vet visit?

Ask a veterinary staff member to contact us at (216) 920-3051 with the name of the person associated with the card.

Your Membership ends on March 31st

What if there’s still money on my Voucher Card?

Any remaining funds are removed from your Voucher Card when your Membership ends on March 31st

What if I still need vet care for my dog or cat?

Renew your Membership. If you qualify to be a renewing Member, you will be eligible to receive $250 on the Voucher Card. The number of Voucher Cards is limited, so act quickly once you receive notification to reapply.

Do I automatically receive a new Voucher Card next year?

No. You must renew your Membership each year.

How do I renew my Membership?

To renew your Membership, you must reapply. We will let you know when the renewal cycle is open.

You will need to confirm or update your contact information, as well as provide proof of financial need for the renewal period. 

What is a Veterinary Partner?

A Veterinary Partner is a vet clinic that provides veterinary care for dogs and/or cats and they accept Veterinary Care Voucher Debit Cards as payment.

A list of Veterinary Partners is provided for your convenience. They are listed by county and then by business name in alphabetical order.  Look for your county and find a location close to you. Not all veterinary services or supplies are available at all locations.

What veterinary care is covered by my Voucher Card?

You can pay for most goods and services that are medically or surgically necessary for your pet's health (or for your health benefit). 

APPROVED goods and services include the following.
  1. Physical exams, vaccinations, parasite control
  2. Spay/neuter procedures
  3. Medically required grooming or dental procedures
  4. Medications or prescription diets purchased in the vet's office
  5. Disease testing and other diagnostics
  6. Chronic care treatments
  7. Hospice or palliative care
  8. Euthanasia

What veterinary care is NOT covered by my Voucher Card?

You may not use the Voucher Card for anything that's not medically or surgically necessary.

Here are examples of what's NOT covered.

  1. Microchipping is NOT covered
  2. Necropsy procedures are NOT covered
  3. Non-prescription diets are NOT covered
  4. Non-prescription pet supplies (e.g., collars, leashes, toys, etc.) are NOT covered
  5. Paying off old debt is NOT covered
  6. Pet adoption fees are NOT covered
  7. Tail docking and ear cropping to meet breed standards are NOT covered
  8. Online purchases, even if it's through your veterinarian's online portal, are NOT covered

What if my vet bill is more than what I have on my Voucher Card?

If your veterinarian recommends a treatment plan that is more than what you have on your Voucher Card (balance checks are free), then you are responsible for the difference. 

We advise you not to spend more than what you can afford. Some diagnostic tests, dental procedures, and surgeries can cost far more than the $250 that is on your Card. Talk to your veterinarian if you can get treatments that meet your budget, even if it means they will need to make their treatment plan without the information they’d like to have.

At the very least, you should seek to reduce pain and distress for your pet, and euthanasia may be the most reasonable and humane solution.
